Activity Guide Registration

To register for programs and activities at the Anthem Community Center, please complete the Program Registration Form below and return it to the front desk at the Community Center or register online by clicking on the following link: Activity Guide Online Registration.* Should circumstances prevent your participation, or you are not satisfied with a program, please contact the Community Center at 623-879-3011. The Community Center can offer another program, or if you prefer, a refund can be initiated after completing the below Refund Request Form.
